Job description

An exciting opportunity has arisen for a dynamic, forward-thinking, and self-motivated individual to join our Community Response Team based over 2 sites in Leamington and Stratford.

Schedule

The Community Response Team operates from 08:30 to 21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, offering a mix of early, late and long days.

Role Overview

As a Community Nurse working in CRT you will assess, identify, and manage patients’ needs who have experienced a deterioration in health. You will handle a varied caseload, seeing patients with long-term conditions, falls, acute illness, providing support at the end of life, and delivering holistic, comprehensive care in their own homes. The aim is to prevent hospital admissions and facilitate timely discharges.

Collaboration

You will work closely with the multidisciplinary team including therapists, GPs, social services, urgent response and others. Collaboration across community service teams ensures seamless care across the locality, within an unpredictable and unscheduled workload.
