Job summary

A health and social care organization is seeking a Community Nurse in Wisbech to provide high-quality nursing care in patients' homes. Job Overview

Are you looking for a new challenge, do you want to be part of a team striving to make a difference with patient care in our community? We are looking for a Community Nurse to work in and around the Wisbech locality. You will be fully supported to gain new skills and experience needed to deliver high‑quality nursing care to patients in their own home or residential setting.

Key Responsibilities
  • Keeping the patient at the centre of care, ensuring patient and carer participation in decision making.
  • Providing a high standard of care and high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required.
  • Working as part of a 24/7 service and multidisciplinary team.
  • Travel independently around the county, holding a full UK driving licence and access to a vehicle (public transport is not permitted).
Additional Responsibilities
  • To work as an autonomous practitioner with the integrated multi‑professional team.
  • To be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines.
  • Provide support and education of peers, new staff, non‑registered staff, and students.
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • Provide high standards of nursing intervention within a patient's own home, including lone working.
Qualifications & Experience
  • Experience of working in the Community.
