Job summary

Surrey Downs Health and Care in Leatherhead is seeking a Band 6 Nurse to join the Leatherhead PCN team. The post focuses on the continuing management of patients and staff within their clinical setting, alongside the team lead.

You will assess care needs, develop, implement and evaluate programmes of care, supervise staff, and facilitate educational activities in partnership with the Learning & Development Team. A driving licence and access to a vehicle are required.

Qualifications

• Must hold a current and valid UK driving licence and have access to a vehicle for work.
• Experience in community-based nursing and post holder to work with PCN teams.
• Band 6 nurse level responsibility with autonomous practice alongside team lead.

Responsibilities

• Manage patients and staff within the clinical setting alongside the team lead.
• Assess care needs and develop, implement and evaluate programmes of care.
• Facilitate educational programmes for training in partnership with Learning & Development.
• Support patient-focused, research-based practice within Surrey Downs Health and Care objectives.
