A healthcare trust in East London is seeking a Band 5 Community Nurse for the Extended Primary Care Teams in Newham. The role is suitable for both experienced and newly qualified Registered Nurses (Adult) looking to enhance their skills in community health. You must be registered with the NMC (Registered Nurses Adult), have some post registration experience and be looking for a career in the community. Final year students waiting for NMC registration will be considered if you meet the other essential criteria in the person specification.

The service is based in East Ham Care Centre and is operational 365/366 days a year including weekends and Bank holidays from 8am 8pm.

The post holder will be responsible for the planning, implementation, and evaluation of individualised nursing care in the community.
The post holder will provide health education to enable patients to self-manage, and achieve optimum independence following deterioration or a period of ill health.
To participate in the training of students, the induction of new staff and others that have contact with our services.
The post holder will be required to support the senior nursing team in the maintenance of efficient and effective delivery of services.
Use a broad range of nursing knowledge, interventions and skills, to prevent, where possible and appropriate, an unnecessary admission to hospital.
Good communication links with patients and carers and joint working with other services across professional and organisational boundaries in order to provide a patient-centered, holistic and seamless service.
