Registered Nurse Associate - Home Visits

Location: Community

Hours: Full-time

We are seeking a compassionate and motivated Registered Nurse Associate (RNA) to join our community nursing team, delivering high‑quality care to patients in their own homes. This role is ideal for a nurse who is passionate about person‑centred care and supporting individuals to remain independent within their community.

You will work as part of a multidisciplinary team, carrying out planned home visits, supporting Registered Nurses, and contributing to the ongoing assessment and care of patients with a range of health needs.

Responsibilities
  • Deliver safe, effective, and compassionate nursing care during home visits
  • Support patients with long‑term conditions, post‑discharge care, and wound management
  • Undertake clinical tasks in line with your scope of practice and training
  • Monitor, record, and report patient conditions and **escalate concerns appropriately**
  • Maintain accurate and timely clinical documentation
  • Work collaboratively with Registered Nurses, GPs, and other healthcare professionals
  • Promote patient dignity, independence, and wellbeing
About You
  • Registered Nursing Associate (RNA) with current NMC registration
  • Experience in community, primary care, or home visiting is desirable (but not essential)
  • Strong communication and interpersonal skills
  • Ability to work independently and manage a community‑based caseload
  • Compassionate, reliable, and committed to high standards of care
