Are you a compassionate, proactive, and skilled nurse looking to make a real impact in people's lives? We are currently seeking a Community Nurse to join our team, providing high-quality nursing care to patients in their homes and community settings across Spalding, Stamford, Market Deeping and Bourne!

About the Role:
As a Community Nurse, you will play a vital role in delivering patient-centred care to individuals with a wide range of health needs. You'll work independently and as part of a multidisciplinary team to assess, plan, implement, and evaluate care plans tailored to each patient.

Key Responsibilities:

Provide nursing care to patients in their homes or community clinics

Manage chronic conditions and support early discharge from hospital

Support patients with complex health needs, including wound care, palliative care, and long-term condition management

Collaborate with GPs, social workers, physiotherapists, and other healthcare professionals

Promote health education and self-management strategies

Maintain accurate and timely patient records

What We're Looking For:

Registered Nurse (Adult) with valid NMC registration

Full UK driving licence and access to a car (or suitable alternative transport)

Excellent communication and clinical decision-making skills

Ability to work independently and manage your own caseload

Previous community or district nursing experience is essential
