CNTW NHS Foundation Trust is seeking a Band 5 registered nurse to deliver person-centred mental health care within the Newcastle North and East Community Treatment Team.

You will assess, plan, implement and evaluate care, working in homes, clinics and community settings with a multidisciplinary team, including depot injections and clozapine clinic support.

An exciting opportunity has arisen for a Band 5 registered nurse to work within the Newcastle North and East CTT. The Band 5 Mental Health Community Nurse is responsible for delivering high-quality, person-centred care within CTT, supporting individuals living with a range of mental health needs in a community setting. The role involves working collaboratively within a multi-disciplinary team to promote recovery, independence, and wellbeing, while reducing hospital admissions and supporting early discharge. The post holder will manage a defined caseload, providing holistic assessment, planning, implementation, and evaluation of care in line with evidence-based practice and organisational policies. Care is delivered in service users' homes, clinics, and other community settings, ensuring accessibility and continuity of care. The role also includes participation in depot and clozapine clinics, ensuring safe administration, monitoring, and follow-up in line with local and national guidelines.Main duties of the job
