Job description

Herefordshire and Worcestershire Health and Care NHS Trust

Community Nurse

The closing date is 19 November 2025

This is a Part time post working 10 hours per week, over 2 evenings per week, working between 18:30-23:30 over a 7‑day rota, based at Kidderminster.

This is a role as a Community Nurse, part of the evening & night team. The team is countywide which is centrally based in Droitwich with satellite bases in Worcester, Evesham, Bromsgrove & Kidderminster, visiting patients in their own home, supporting them in remaining at home, preventing admission to hospital & supporting early discharge. This post will be primarily based in the North of the county from either Bromsgrove or Kidderminster.

The post is worked as unsocial hours hence picking up pay enhancements, which makes this post more attractive. More money for less hours!!

These working hours offer perfect work‑life balance particularly if you have a young family; you will have the daytime to enjoy hobbies & activities but remain in a progressive career.

We pride ourselves in providing outstanding care to patients across Worcestershire. We will provide you with resources to develop and progress a long‑term career with flexibility, continual professional development as well as a wide range of wellbeing & support. If you would like to discuss informally if this is the right role for you, get in touch with Recruiting Manager which can be found on the top right‑hand side of the advert.

Please note the Trust is unable to provide sponsorship to work for this role; to apply you must have current right to work in the UK status.

Main duties of the job

As a community nurse working with the Evening and night Team most of the work allocated to you will be of an unplanned nature. The service works as an on‑call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work alone.

The evening service nurses primarily work alone, they have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will be able to start your visits from home if the geography dictates.

The patients that are seen during the evening and night vary immensely and no 2 shifts are the same. For example, we see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention.

As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to urgent Care response. These can be patients that have fallen, or patients requiring further assessment to aid prevention in admission to hospital.
