Role Overview

An exciting opportunity has arisen to join us as a Community Nurse, providing care to patients with complex care needs and enabling patients to receive the care they need in their own home. We have two roles available – one in the Petersfield Community Team and another in the Fareham Community Team. Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload for a group of GP Practices as part of the Integrated Care Team.

Responsibilities

The post holder will have responsibility for managing, coordinating and delegating work, ensuring referrals are dealt with according to clinical need. Holistic assessments and care plans are recorded in line with Trust and professional record‑keeping standards, and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and to support timely discharge into the community. The post holder will also have a responsibility for re‑enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

  • Ability to work autonomously and support the Team Lead in managing case load and staff development
  • Proactive and positive attitude to learning the skills required to fulfil the role
  • Access to a vehicle and possession of a full driving licence
