A healthcare service provider in Basildon is seeking a motivated Band 5 Community Nurse to join their Integrated Care team. The role involves patient care delivery, supervision of staff, and collaboration with health professionals. Candidates should be registered nurses with NHS experience. We are seeking a motivated and dynamic full‑time Band 5 Community Nurse to work within the Integrated Care team in Basildon and Wickford. The Basildon and Wickford ICT is a forward‑thinking, innovative, community‑based service that works in a fully integrated way. The team includes physiotherapists, occupational therapists, nurses, community matrons and care co‑ordinators working side by side to provide a holistic approach to care.

The post holder will be a registered nurse with NHS experience, and community experience is desirable but not essential.
The post holder will be responsible for patients allocated to them and will assess, manage, plan, and deliver care. They will supervise pre‑registration students and Band 4 nursing assistants/assistant practitioners. They will support peers and senior nursing staff, be an effective team member and work on their own initiative. They will actively collaborate and liaise with other health and social professionals and agencies. The role requires computer literacy as all patient records are electronic.

This post attracts a high‑cost‑area supplement of 5% of the basic salary, with a minimum of £1,303 to a maximum of £2,198 per annum pro‑rata for part‑time work.
