Job summary

Epsom and St Helier University Hospitals NHS Trust is advertising a Band 6 Specialist Community Dietitian to join the Neuro-Rehabilitation Service within Surrey Downs Health and Care. The post involves delivering dietetic care in the community, supporting enteral feeding, and working with a multidisciplinary team.

The role requires HCPC registration and experience with neurological conditions, plus a valid UK driving licence for home visits.

Qualifications

• HCPC-registered Dietitian with post-registration experience.
• Experience with patients with neurological conditions, including stroke.
• Strong communication, clinical reasoning and team-working skills.
• Confidence in managing enteral feeding and ethical issues in community setting.
• Motivation to contribute to service improvement and audit activities.

Responsibilities

• Maintain a clinical caseload in neuro-rehabilitation within community settings.
• Undertake comprehensive assessments and develop nutrition care plans.
• Collaborate with the Prescribing Support Dietitian for cost-effective use of enteral nutrition.
• Deliver high-quality dietetic care to patients in homes, care homes and clinics.
• Contribute to policy, guidelines and service development.
• Support NICE guidance and local care pathways implementation.
• Ensure timely documentation and participate in audits.
• Provide flexible service delivery and cross-team collaboration.
• Stay updated with current literature and share learning.
• Ensure risk and other assessments are completed per Trust policies.
