Aneurin Bevan University Health Board (ABUHB) and Blaenau Gwent County Borough Council are working in partnership to create an exciting new role within the Community Resource Team (CRT).

This is a fantastic opportunity to play a vital part in shaping how care and support come together across Blaenau Gwent. As a Community Multidisciplinary Coordinator, you’ll help build a seamless, person-centred approach to wellbeing — connecting professionals, improving communication, and ensuring citizens receive the right care, in the right place, at the right time.

About the Role

As a Community Multidisciplinary Coordinator, you’ll play a key role in bringing health, social care, and third-sector professionals together to plan and deliver coordinated, person-centred support for our citizens.

You’ll organise and lead multidisciplinary team (MDT) meetings — preparing agendas, capturing actions, and ensuring follow-up tasks are completed. You’ll be the vital link that keeps communication flowing between partners, ensuring that decisions made in meetings translate into timely, practical support for citizens.

Alongside this, you’ll help monitor performance, analyse data, and identify opportunities to enhance the way we work. You’ll be encouraged to use your initiative, bring fresh ideas, and help shape a more connected, efficient, and compassionate community service.
