About this role

We are looking for a Mental Health Support Worker to join our Crisis Resolution and Home Treatment Service Team based at Haywood Lodge, St George's hospital in Stafford. The multi-disciplinary team provides a 24/7 service to the population aged 16 and above residing in the Southwest Staffordshire Locality, delivering urgent response and a comprehensive Mental Health Assessment to people presenting in an acute Mental Health Crisis, and Home Treatment to those requiring a period of short‑term intensive intervention.

Under the direction of senior health and social care staff and/or the shift coordinator you will assist in delivering interventions with an allocated group of service users with mental health needs. Interventions will focus on assisting the person to identify and achieve their agreed goals of recovery, meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living and mental health promotion, supporting the service user to complete outcome measures and participate in care reviews.

Key responsibilities
  • Deliver a high-quality clinical service to patients and their families, including accompanying Band 6 staff in crisis assessments, assisting with effective home treatment to prevent hospital admission, and conducting physical health assessment and care.
  • Work collaboratively with other Trust services to provide seamless care along defined care pathways.
  • Perform lone working as part of the role.
  • Work in partnership with external agencies to support service‑users’ access to services in the independent and non‑statutory sector.
  • Work shift patterns covering 24/7; required to work night shifts.
  • Support service users to complete appropriate outcome measure tools and participate in reviews of care and effectiveness of the interventions provided.
Requirements and working conditions

You will need to travel independently over a large geographical area to see service users in their homes, so you will need access to suitable transport.
