This advert is for vacancies for the Camberwell and Peckham Team based at St Giles CMHT.

Key Responsibilities
  • Significant knowledge, awareness and understanding of Mental Health Law, including legislation relevant to the community sector including the Community Care Act (2015) treatment legislation, the Mental Capacity Act (2005) and Community Treatment Orders (CTO).
  • Be personally responsible and professionally accountable for a caseload as part of the community team and lead and manage the work of others as required.
  • This post will primarily be based at St Giles House, CMHT, with visits in the wider community to other agencies and clients’ homes as required.
  • The Care Coordinator will manage a caseload of clients (and provide support to their carers) with complex needs and varying diagnoses.
  • Support the client’s journey during their time under CMHT care, including leading clinical discussions and supporting the pathway out of the CMHT.
  • Act as part of the interface between partner agencies to ensure timely and effective transition and discharge into the wider community.
  • Work cohesively as part of the wider community team in managing the larger caseload by supporting on Duty and contributing positively in team meetings.
  • Maintain good IT skills and up-to-date documentation and relevant assessments on patient electronic records.
  • Coordinate care, communicating with other professionals involved in the care and ensuring regular reviews; attend ward meetings and interface with services such as Addictions and CAMHS where appropriate.
  • Independently carry out assessment, care planning and care coordination with excellent interpersonal skills; listen to and value individuals from diverse backgrounds.
  • Lead meetings with the community team to discuss, report and evaluate client care; maintain a flexible approach focused on choice and social inclusion.
  • Recognise and address physical health needs of service users with long-term conditions; escalate as necessary to senior MDT members.

Additional Responsibilities
  • Promoting Recovery: empower service users to take responsibility for their care with recovery-focused approaches; support wellbeing, independence and self-care.
  • Risk Assessment and Risk Management: report incidents, maintain accurate records, and manage medicines and equipment safely; follow Trust policies and safeguarding guidelines.
  • Safeguarding and Duty of Candour: understand and implement safeguarding; uphold Duty of Candour in the community context.
  • Infection Control and Medication Management (for nurses): administer and monitor medications per NMC standards; ensure correct administration and storage.
  • Administration and Leadership: maintain complete electronic records; write reports and support clinical supervision and junior staff; demonstrate leadership within the community setting.
