Main duties of the job

Job responsibilities

Job Purpose

The post holder will deliver high quality mental health services, The role involves managing adefined caseload. The focus will be providing continuous assessment and treatment to peoplewith mental health difficulties and their carers. This will include recovery co-ordination and caremanagement functions, liaison with statutory and non-statutory organisations. The post holderwill be an effective member of the multi-disciplinary team.

Duties and Responsibilities

Communication and Working Relationship Skills.

Contribute to the teams engagement including liaising with a wide range of otherprofessionals and agencies.

Impart complex and sensitive information to :

Patients and families/carers re assessment, care planning, treatment and review.

Multi disciplinary teams to ensure the provision of consistent well coordinatedcare.

Primary care teams regarding client care.

External agencies also involved in the provision of care e.g.: voluntary andprivate sector, police, social services, housing, employment etc.

Service user/carer groups

Contribute to the safe and effective day to day operations of the service including thehandling of clinical enquires.

Provide and receive information some of which may be contentious, sensitive andcomplex to and from individuals and their families/carers and actively seek support asappropriate.

Provide a high standards of written and verbal communication that is clear and in linewith professional documentation standards.

Participate in clinical meetings,, this will include CPA and non-CPA reviews, professionalmeetings, and clinical risk meetings.

Communicate information in a way that makes it relevant and understandable for serviceusers and carers, working in line with practice standards and operational policies.

Actively contribute to multi-disciplinary team working.

Facilitate fair access to social care services and promote social inclusion regardless ofage, ethnicity, gender, sexual orientation or disability.

Analytical and Judgemental Skills

Undertake risk assessments and determine the appropriate course of action, which hastaken into consideration a range of possible options. Where necessary, ensure thatconsultation with senior colleagues is undertaken.

Undertake and continuously re-evaluate bio-psycho-social assessments and develop aworking formulation and diagnosis which will lead to the development of personalisedcare packages in line with evidence based and payment by results.

Planning and Organisational SkillsOrganise meetings, including CPA and non-CPA professional and risk meetings.

Responsible for managing own workload to ensure the needs to the individuals who use our services are met, taking the wider needs of the team into consideration.

Ensure that interventions are planned in an integrated and co-ordinated manner, thismay include liaison with external agencies.

Plan and organise individuals packages of care and ensure that reviews are held withinthe required time frames, for example Section 117, Funding Panels and CPA.

Plan their own workload and the workload of junior colleagues (e.g. Band 3 colleagues)to ensure that care is provided in the most high quality and cost efficient manner.

Responsibility for Patient/Client Care, Treatment and Therapy

Ensure that designated caseload is well managed and that care and treatment isprovided in line with practice standards and recovery co-ordination (CPA and non-CPA ).

Understand social factors that impact upon mental healthDeliver care packages including assessing, planning, intervention and evaluation.

Ensure that all interventions have clear outcomes and that these are taken intoconsideration when sourcing the resources to best meet the individuals needs. This willinclude enabling individuals to take up Direct Payments and offering the person as muchchoice and control of resources as appropriate.

Ensure that all social care packages are reviewed within the relevant timescales andfocus on integration within existing community resources where possible.

Deliver services using a recovery and strengths approach where the needs of theindividual and their carers are held as central and their involvement is proactivelyencouraged.

Deliver care in line with evidence based practice and employ engagement andtherapeutic skills that are appropriate to the clinical situation presented.

Provide specialist advice to referrers/other multi-disciplinary staff in relation to the care ofindividuals.

Enable communication and engagement of individuals and their carers to ensure thatthey have a voice about the services they receive and how these are developed. Thismay include engagement of advocacy services.

Ensure that appropriate action is taken to safeguarding adults and children. This willinclude attending safeguarding meetings and implementing relevant care plans under theguidance of a Senior Mental Health Practitioner.

Ensure that assessment and treatment plans are holistic, outcome focused and take intoaccount the physical health and social needs of the individual, including input ofsignificant others.

Ensure the monitoring and review of mental health medication taking into considerationinteractions with physical health needs.Ensuring timely and accurate documentation of all clinical activity in line withprofessional guidance and best practice.

Practice in a way that actively minimises dependency and promotes recovery.Contribute to clinical governance arrangements in order to ensure the quality of theservice provided, this will include ensuring performance targets are met to provide highquality care.

  • Postgraduate experience in at least two inpatient settings or substantial experience working within a mental health and/or
  • Managing a caseload of clients with a range of mental health problems.

  • Working within a community setting in either the statutory or voluntary sector.
  • Providing clinical supervision.
  • Experience of working in a multi-disciplinary and multi-agency environment.
  • Demonstrates experience of, and relevant professional practice qualification in mentoring/assessing students and learners.
