Job summary

Eastleigh Southern Parishes CMHT team based at Moorgreen, West End Totton, near Southampton, is inviting applications for a full-time Band 6 Mental Health Practitioner. You will manage a caseload of individuals with complex, long-term mental health needs and apply strength-based recovery methods to improve outcomes.

NHS policies and practices will guide your work within a supportive team. The role requires proactive, innovative thinking and a commitment to high-quality holistic care for adults

Qualifications

• Broad experience treating adults with severe and enduring mental illness.
• Experience in assessing and formulating intervention plans and outcome measures.
• Knowledge of NHS policies and practices in mental health.

The Eastleigh Southern Parishes CMHT team based at Moorgreen, West End Totton, near Southampton has an exciting opportunity for a full-time Band 6 Mental Health Practitioner.

If you're innovative, proactive, and passionate about providing high-quality holistic care, then we would love to hear from you. You will have a broad depth of mental health experience working with individuals who have severe and enduring mental illness.

You will be responsible for care-coordinating a caseload of service users with complex long-term mental health problems who find it difficult to engage with services. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.
