South West Yorkshire Partnership NHS Foundation Trust is seeking an experienced Mental Health Nurse to join the Enhanced MH Team West in Barnsley. You will work within the Flexible Assertive Community Treatment (FACT) model to deliver high quality community care.

You will manage a caseload of around 25 people, arrange care, review care plans and risk assessments, and liaise with other professionals and agencies to keep service users and their carers at the centre of everything we do.

Qualifications

  • Registered MH nurse with current NMC registration.
  • Experience delivering FACT or similar community mental health care.
  • Strong assessment and risk management skills.

Job description

The successful candidate will manage a caseload of around 25 people, they will be responsible for arranging care, liaison with other professionals and agencies, manage and review care plans and risk assessments ensuring these are done collaboratively in a recovery focused manner, keeping the person and their carers in the centre of all we do.

The team use the Flexible Assertive Community Treatment Model, and we have a daily shift coordinator on a rota system which will require the successful candidate to delegate and prioritise work, oversee and facilitate the daily morning meetings, managing the team diary and carry out any urgent visits.

The successful candidate will need to be an efficient organiser, managing their workload and be flexible to meet the needs of service. They will need to manage their own requirements for supervision in line with their professional registration.

We are multidisciplinary team that provides high quality face to face care utilising the FACT model (Flexible Assertive Community Treatment) this is central in to keeping people well and out of hospital.

The successful candidate would be the key person in the service users recovery journey working with other agencies and alongside family & carers keeping the person in the centre of all we do.

We offer flexible working hours between 08.30 - 19.00 Mon to Fri & 9-5 on weekends & BH. We are family friendly & all staff have agile working equipment. We are open 365 days a year & pride ourselves on excellent team working.
