Overview

Lancaster and Morecambe Community Mental Health Team (CMHT) is recruiting a qualified Band 5 Mental Health Nurse to work predominantly within our Physical Health Monitoring Team. The role is permanent, 37.5 hours per week (part‑time applications considered). Applicants must be registered with the NMC.

Role

The post involves performing duties as a Community Mental Health Nurse and undertaking assessments for people referred into the service for physical health monitoring when commenced on antipsychotic medication, including depot and Clozapine. You will develop care plans and deliver evidence‑based interventions, manage risks in the community for patients under the CMHT for shared care, and will work under the supervision of a Band 6 Community Mental Health Nurse. You may also have the opportunity to supervise junior staff.

This role involves managing unplanned patient work, responding to escalation of risk/crisis situations and stepping up care to a CMHT Band 6 Practitioner when required.

We provide clinical supervision, management supervision, team/peer support, internal and external training opportunities, the Trust’s appraisal process, and, for newly qualified nurses, a preceptorship programme.
