Job summary

Oxford Health NHS Foundation Trust is seeking a qualified Mental Health Nurse to join the Wantage and Faringdon AMHT, based at Wantage Community Hospital. You will assess new patients, provide follow-up, and help with discharge planning in collaboration with inpatient teams.

Responsibilities

• Assess new patients referred into the AMHT.
• Provide brief follow-up work after assessment.
• Support discharge planning with inpatient services.
• Hold caseloads for longer-term follow-up.
• Liaise with multi-professional and multi-agency teams.
