Job summary

Hertfordshire Partnership University NHS Foundation Trust in Watford is recruiting Band 6 Community Nurses to join our Adult Community Mental Health Services in Dacorum. The role involves assessing service users, developing personalised care and support plans, carrying out risk assessments, and monitoring physical health, medications, and responses.

Responsibilities

• Assess needs of service users and formulate personalised care and support plans with service users and families.
• Conduct risk assessments and monitor physical health, including blood pressure, glucose, and weight.
• Administer oral and injectable medications, including storage, transportation and monitoring of responses.
• Collaborate with multidisciplinary teams and allied resources to review intervention effectiveness and re-plan as needed.

The post holder will be able to undertake assessments of needs of service users, working in partnership with service users and their families, to formulate Personalised care and support plans and risk assessments.

The post holder will work with colleagues and allied resources to review the effectiveness of interventions and re-plan using evidence based interventions to best effect in partnership with the service user.

The post holder will be involved in physical procedures including administration of oral and injectable medications, the storage and transportation of them, and the monitoring of people’s physical responses to medications and physical health, such as blood pressure, glucose and weight monitoring.
