A national health service organization is seeking a Community Mental Health Nurse to join their Community Interface Team in Hartlepool. The ideal candidate will support adults with mental health needs, performing assessments and interventions in a collaborative environment. An exciting opportunity has arisen for a Community Mental Health Nurse to join our Community Interface Team based in Hartlepool. The role involves supporting adults living with mental health needs, the service offers initial assessment and short-term interventions within the community setting with the primary aim of promoting both recovery and living well, liaison with partner agencies to signpost and utilise other resources to support the patient’s individual needs.
Within Hartlepool Community Interface Team we have mixed roles, from Triage, Tel and Face to Face Assessments, One to One ungoverned interventions and delivering groups, but also working alongside our patients with enduring mental health problems and long term medication.
This is an ideal opportunity for someone who wants to develop into a community role within a team of supportive and experienced professionals. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
