A healthcare organization in the UK is seeking a Registered Nurse to support older adults in Hastings & Rother as part of a community service. The role involves managing complex cases, conducting assessments, and providing essential support. Candidates should have significant experience in mental health care and be committed to professional development. As part of the SOAMHS team you will be support the older adults in Hastings & Rother to remain as independent as possible and remain in the community.
You will be supported by colleagues and management in your role.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).
To provide comprehensive assessments of service users with identified, complex needs.
To manage a caseload of our service users with complex mental health needs, and undertake case management/lead professional role as needed.
Offer psychoeducational, coping skills and support through a period of stabilisation as needed
This is a community based service, providing a support to SOAMHS service users across Hastings & Rother.
To be able to communicate with and meet the needs of our service user group, their families and carers and other agencies involved in their care
To have experience of and specialist knowledge of interventions for older people who can have some additional needs, and to be able to lead on clinical initiatives with an emphasis on continual improvement.
To receive management supervision and continued professional develop.
Support the Trust's visions and values and in particular the promotion of a positive approach to diversity, equality, rights, and treating others with dignity and respect; to eliminate discrimination and disadvantage in service delivery and employment, and to manage, support or comply by adhering to the Trust's Equality and Diversity Policy.
To monitor, risk progress and where necessary compliance including the service users views to their care and treatment and when appropriate to work in partnership with the service users family and carers.
Develop professional networks and keep up to date with locate resources.
Establish robust communication networks with clients' carers and other health workers and agencies.
Promote awareness of the professional role within the team and across the service.
