Community Mental Health Nurse
Band 6 - £38,682 - £46,580 per annum
37.5 hours per week
Location: Taylor Centre, Warrior House, Southend
Contract: Fixed Term Contract/Secondment until 30 March 2026

We are currently looking for an experienced, enthusiastic and dynamic Care Coordinator for a 5‑month period. As the successful post‑holder you will manage a caseload of service users with complex health and social care needs, following a recovery‑focused model. Previous care coordination experience is essential for this post.

Main duties of the job
- Assessment, planning, implementation and evaluation of evidence‑based care, including health promotion for named service users/groups.
- Admission, case conference/review and discharge in collaboration with the multi‑disciplinary team.
- Coordinating care and assessment under the Care Programme Approach for a designated caseload.
- Ensuring compliance with Safeguarding Vulnerable Adults and Children legislation.
- Ensuring adherence to the Mental Health Act 1983.
- Promoting carer and service user involvement within the service.
- Establishing therapeutic relationships and implementing evidence‑based therapeutic interventions within boundaries set by the Nursing and Midwifery Council code of conduct.
- Providing assessments, education and support to carers in line with NSF requirements.
- Deputising and providing support and caseload coverage for colleagues/team members.
Additional responsibilities
- Carry out mandatory training in line with Trust policy and procedures.
- Participate in the staff appraisal process and manage staff if applicable.
- Keep updated on Trust policy matters.
- Provide management supervision when appropriate.
- Work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi‑disciplinary team.

Person Specification
- Post‑2‑year appropriate mental health nursing qualification / experience.
- Experience working within community mental health teams/services.
- Understanding of responsibilities under the Mental Health Act, Care Act and Mental Capacity Act.
- Must be able to drive and have access to a vehicle for work purposes.
