Job description
Key Responsibilities
  • Assessment, planning, implementation and evaluation of evidence‑based care for designated service users or groups, including health promotion.
  • Admission, case conference/review and discharge in collaboration with the multi‑disciplinary team.
  • Coordination of care and assessment under the Care Programme Approach for a designated caseload.
  • Ensuring compliance with Safeguarding Vulnerable Adults and Children legislation and with the Mental Health Act 1983.
  • Promotion of carer and service‑user involvement and facilitation of their participation in the service.
  • Establishing therapeutic relationships with service users and delivering evidence‑based therapeutic interventions, observing appropriate boundaries in line with Nursing and Midwifery Council codes.
  • Ensuring carers complete assessments, education and support in accordance with NSF requirements.
  • Deputising and providing caseload coverage for colleagues or team members.
  • Clinical risk assessment of service users contacting the “Out of Hours” service, delivering advice and decision‑making, and coordinating any reviewed care plans.
  • Participation in service development, setting quality standards, and auditing, monitoring and reviewing activities in line with current clinical guidance and policy.
Key Activities and Additional Duties
  • Complete mandatory training in line with Trust policy and procedures.
  • Participate in the staff appraisal process and undertake appraisals for staff you manage.
  • Stay updated on all matters relating to Trust policy.
  • Provide management supervision where appropriate.
  • Cover training and up‑skilling in the teams you support.
  • Actively support DBS completion and related compliance.
