Are you passionate about supporting working aged adults to have good mental health? Are you a recovery focused and creative mental health Nurse or newly qualified Nurse and want to work in a supportive and well established multi-disciplinary team? An opportunity has become available to work in Goole Community Mental Health Team.

This post will be based at Goole CMHT Bartholomew House

Main duties of the job

As a key worker / care coordinator you will provide support and interventions to promote mental health recovery. Establishing therapeutic relationships with service users and their family / carers.

  • Develop and deliver recovery focused care in collaboration with service users and their family /carers.
  • Provide information and psycho education surrounding mental health conditions and medication.
  • Uphold legal frameworks which underpin service delivery.
  • Assess risks, manage risk and the effectiveness of interventions.
  • Signposting / referral to external services when required.
  • Develop good working relationships with neighbouring teams to enable effective co-working and responsive transfers of care if necessary.
  • Act as duty officer on a rota basis
  • Act as a Care coordinator for a caseload of clients
  • Participate in Group work
  • Support the administration and monitoring of medication.

Person Specification
Qualifications and knowledge
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion
  • Part III or 13 of NMC live register
  • Evidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/chang e that impacts within own area of work/responsibility
  • Evidence of CPD or experiential learning at an advanced level e.g. Post graduate/Masters level
Experience
  • Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
  • Act as Care Programme Approach (CPA) Co -ordinator
  • Ability to effectively Chair meetings
  • Demonstrable experience of working in the specific field where the post is held
Skills and Competencies
  • Working knowledge of Trust policies and procedures in order to effectively lead and manage others
  • Ability to lead and motivate staff to embrace change
