Community Learning Disability Nurse

An exciting opportunity has arisen for an experienced Learning Disability Nurse to join the Adult Community Learning Disability Team in Fylde and Wyre.

The Learning Disability Service strives to provide forward-thinking high-quality services, working in partnership with other agencies. In addition to working with adults with learning disabilities and autism, we work closely with their families, and we have a strong commitment to health promotion through service development and training initiatives. We are committed to helping our staff develop and you will be part of an enthusiastic and experienced team.

We are looking for a registered nurse with an up-to-date knowledge of health policy for people with learning disabilities including knowledge of the Transforming Care agenda. Adaptability, good communication skills and self-motivation are essential. The successful applicant will have proven skills in working with people who have complex health needs and challenging behaviour. An interest in one of the following areas would also be advantageous: Positive Behavioural Support, mental health, forensic issues, and acute/primary care liaison.

Main duties of the job

The post holder will work within an integrated health team, providing clinical support to adults with learning disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues. The post holder will play an integral role in delivery of care within these pathways within a variety of community settings.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
