A reputable NHS organization is seeking a part-time Learning Disability Nurse to join its Community Learning Disability team in Blackwood. The role requires maintaining a clinical caseload, conducting assessments, and collaborating with multi-disciplinary teams to support adults with learning disabilities. The postholder will be a member of the Community Learning Disability team, working closely with the multi‑disciplinary team. Responsibilities include maintaining a clinical caseload and conducting assessments of highly complex problems which require the analysis, interpretation and comparison of a range of solutions for adults with learning disabilities and their families who are experiencing complex health difficulties associated with their learning disability. This role will include maintaining clinical standards within the governance framework.
Work within the code of practice to take responsibility for the caseload, ensuring comprehensive assessments of needs. This includes the formulation, planning, delivery and evaluation of care and involves the regular review and updating of risk assessments, assessment documentation and care plan reviews. Work collaboratively across a wide range of multi‑disciplinary services and agencies to provide effective delivery of care, supporting a programme of equal access to mainstream services with reasonable adjustments and collaborative interventions from learning disability services. Engage in service development and embrace new initiatives, supporting service outcome measures using clinical and service audit. Contribute to the development and implementation of learning disability strategic planning and service development within the Learning Disability Directorate.
