Job summary

Coventry and Warwickshire Partnership NHS Trust is seeking a Community Learning Disabilities Nurse (RNLD) to work in North Warwickshire within a Community Learning Disabilities team. The role involves managing a caseload, completing comprehensive assessments, and providing specialist nursing advice in the community, while collaborating with multi-disciplinary colleagues.

The successful candidate will be proactive, self-motivated, with autonomous working experience and current NMC registration,

Qualifications

• Substantial post registration experience as a Band 5/6 nurse working with people with a learning disability
• Experience of working with people with a learning disability in the community
• Driver with access to independent means of transport

Responsibilities

• Specialist assessment of client needs and development of care programmes
• Manage a caseload and act as health facilitator for people with a learning disability
• Promote needs and rights to other agencies and the public
• Develop new services within the community to meet identified needs
• Collaborate with the Learning Disability Nursing Team and other professionals
