Job summary

Look Ahead Care, Support and Housing Ltd is seeking a compassionate Specialist Support Worker, Community Integration lead for our Forensic Mental Health service in Kingston. You will provide high-quality specialist support and lead the service's approach to community reintegration.

You will manage a small keyworking caseload, develop risk and progression plans, and build strong relationships with customers and local organisations to create real routes into community life.

Qualifications

• Experience supporting adults with complex mental health, social care, housing or criminal-justice needs.
• Specialist forensic mental health knowledge.
• Relational security and formulation-informed support.
• Knowledge of Kingston and South West London resources.

Responsibilities

• Hold a small keyworking caseload and co-produce support, risk, crisis, relapse-prevention and progression plans.
• Build strong, boundaried relationships with customers who may be ambivalent, anxious about change or difficult to engage.
• Map and build relationships with community organisations, ensuring real routes into local life.
• Assess barriers to community participation, including confidence, institutionalisation, stigma, transport, finances, social anxiety, statutory restrictions and risk-related concerns.
