Job summary

The City of Edinburgh Council is seeking a Local Area Coordination Team Practitioner in Craighall Centre, Edinburgh. Salary £30,188 - £34,452, 36 hours per week, 52 weeks.

You will provide short term preventative support to help people with learning disabilities connect with their local community and access opportunities that improve quality of life. Qualifications

• SVQ Social Services and Healthcare SCQF Level 7 and HNC Social Services (or equivalent).
• Understanding of inclusion and active support principles.
• Safeguarding knowledge with risk assessment.
• Ability to plan and prevent issues, promote independence.
• Knowledge of local services and community resources.

Responsibilities

• Encourage and enable people with learning disabilities to live an active, more connected life in their community.
• Hold discussions to determine needs, strengths and goals and develop plans with the person.
• Support independence and provide coaching and practical or emotional support in areas like employment, education, health, transport and relationships.
• Implement informal programmes of support and monitor outcomes reporting to the Local Area Co-ordination Supervisor.
• Promote inclusive practices and improve accessibility and participation in the community.
• Encourage health and self-care and promote personal responsibility.
• Assist in developing partnerships to expand local opportunities and community engagement.
• Model behaviours: Respect, Integrity, and Flexibility in daily work.
• Work with families, carers and professionals to ensure a holistic approach.
• Uphold safeguarding, report concerns, and escalate as required.
• Attend meetings, share insights and help balance safety with independence.
