Community Hospice @ Home Clinical Nurse/Paramedic Specialist
  • Hours: 37.5 per week (30 hours per week can be considered)
  • Location: Rennie Grove Peace Geographical Area, Base: Bucks Locality (Gillian King House, Chalfont St Giles)

Main duties of the job

The purpose of the role is to provide high quality specialist palliative practitioner care, skills, advice and support to patients, carers and those important to the patient. This role will support the delivery of planned and unplanned care across Rennie Grove Peace Hospice.

This role will suit someone who Is an experienced palliative care nurse or paramedic who would like to be part of a community service which aims to ensure equity of access to Rennie Grove Peace Hospice services.

At Rennie Grove Peace Hospice Care, our unique care provides a lifeline of compassion and support to patients, carers, and families facing progressive life-limiting illness. We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.) Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces.

We are proud to offer care in the place that best suits the individuals needs, whether that's at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities. As a hospice care charity, we have offered this support, free of charge, for over 40 years.

Our staff team of 340+ includes a wide range of patient-facing disciplines plus operational support based in Watford, Tring, St Albans, Chalfont St Giles and Berkhamsted. The Charity is now one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

Job responsibilities
  • Actively participate in the development of the organisations planned and unplanned community model of care.
  • Offer advice, support and education to the primary health care team as part of an interdisciplinary team
  • The management of a specialist palliative planned care caseload within a set Primary Care Network (PCN) and cross cover other surgeries as required to maintain our service.
  • To work from an interdisciplinary approach to care.
  • To engage in the full range of activities consistent with the role of a Clinical Nurse Specialist/ Clinical Paramedic Specialist.
  • Demonstrate specialist skills and expertise in palliative care, the post holder will deliver high standards of personalised care.
  • Improve patient outcomes and experience by providing physical and emotional support to their patients, coordinating their care services, and informing and advising them on clinical and practical issues associated with their illness.
  • Participate activities such as audits and case reviews to improve the Hospice @ Home service
Person Specification
Qualifications
  • First level Registered Nurse or Registered Paramedic
  • Degree Level or modules in Palliative Care or another relevant subject
Additional Criteria
  • Evidence of continuous personal and academic development
  • Non-medical Prescriber or a commitment to successfully achieve this qualification as agreed with line manager
  • Advanced assessment skills training/commitment to attend this qualification
  • Advanced communication skills training
  • Proven experience of managing people / strong clinical leadership.
  • Proven experience of effective team working
  • IT literate including a good knowledge of Excel, Word, a clinical IT system.
  • Excellent written and verbal communication skills
  • Good organisational skills
  • Teaching and presentation skills
  • Advanced assessment skills and specialist symptom control knowledge to plan, implement and evaluate nursing care
  • Drive and enthusiasm with a positive approach
  • Ability to work autonomously and as an effective member of the team
