Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting for a Healthcare Support Worker to join our Community Nursing Service across Hampshire in a permanent role. The service operates 8am–8pm, 7 days a week, delivering high-quality, patient-centred care in patients' homes and coordinating with a multi-disciplinary team.

Role involves following care plans, contributing to care planning and communication, and supporting patients to maintain independence.

Qualifications

• Full driving licence and access to a vehicle.
• Ability to work as part of a multi-disciplinary team.
• Will follow written care plans and treatment protocols under supervision by Registered Nurse.

Responsibilities

• Work under supervision following written care plans and care pathways devised by the Registered Nurse.
• Follow prescribed care plans and treatment protocols.
• Follow HPCT Policies and Procedures.
• Report variances to Registered Nurse.
• Provide general administration support to the team.
