Job description

The Community Health Care Support Worker will support the Registered Nurses in all areas of the service's referral criteria. This covers a broad range including Mental Health, Trauma and Emotional Distress, Complex Physical Health, Sexual Health and Relationships, Behaviour Support and Dementia. Key aspects of the role include working as part of a multi‑disciplinary team to respond to nursing referrals and deliver person‑centred care; working under the direction of the Lead or Registered Nurse to implement individual care plans, carrying out assigned tasks and interventions as required; reporting back any findings, changes, or concerns regarding service users' health, behaviour, or environment promptly to the Registered Nurse; contributing to risk assessments, treatment plans, and team reports; promoting safety, dignity and wellbeing in line with Health & Safety policies; managing challenging behaviour using approved methods and care plans; traveling to visit people in their homes and within the community, including areas not accessible by public transport; maintaining effective communication (verbal, non‑verbal and written) with both service users and other professionals; and liaising with professionals, carers and agencies to ensure consistent and coordinated care.

The Team strives to provide forward‑thinking, high‑quality services that work in partnership with other agencies. The post holder will have experience of working with people with a learning disability and an enthusiasm to provide holistic person‑centred care. You will be working as part of a team where you will need to be able to communicate effectively with other staff, patients and their families/carers. The job covers multiple bases across Derby City and Derbyshire, and the post holder must meet the travel requirements to complete patient visits in the community.

A minimum of NVQ 3 or equivalent is essential. Those working towards this qualification will also be considered. The successful candidate can expect to work within a friendly, professional and committed team, receive regular supervision and have training and development opportunities.
