Join Our Caring Community First Team - Community Health and Social Care Support Worker Opportunities Are you passionate about making a real difference in people's lives? We’re looking for compassionate and dedicated Community Health and Social Care Support Workers to join our Community First Team at the Perth House Assessment Hub in Derby.

Working Hours:

You’ll work a 4‑on, 4‑off rota, alternating between:

  • Morning shift: 7:15 AM – 2:45 PM
  • Evening shift: 2:30 PM – 10:00 PM

Shifts include weekends and bank holidays.

Main duties of the job

The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values. To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians. To assist all clinicians in developing agreed programmes of treatment and care where appropriate and to work as an effective member of the wider multi‑disciplinary team. To carry out individual care without direct supervision in the Perth House Assessment Hub. To provide support to patients to optimise their health and independence.
