Job description
Overview

An opportunity has arisen within South East Essex Dementia & Older Adults Community Mental Health Service for a Registered or newly qualified Band 5 Mental Health Nurse.

Responsibilities
  • Work with the Care Home Team to assess needs, support diagnosis, provide ongoing review and care planning for individuals in residential care settings with a diagnosis of dementia or suspected dementia.
  • Collaborate with a range of professionals and link with other services within South East Essex, including the Single Point of Referral (SPOR) and Southend University Hospital.
  • Support people and their carers in residential care settings to ensure high‑quality care, identify deterioration or rising risk of admission, and provide timely intervention, support and specific training to carers.
  • Behavioural management: identify clients with behavioural problems, such as violent or potentially violent behaviour, and ensure other staff are aware of the risks involved, applying caution and care when visiting, organising joint visits and remaining mindful of the lone worker policy.
  • Act as an ambassador for EPUT within the work setting, undertaking assessments where required, including planning, facilitating, implementing, and evaluating care for individuals with dementia to avoid unnecessary hospital re‑admission and to provide ongoing support to the wards within the hospital.
  • Advise a range of health and social care professionals on effective treatment, interventions and management of individuals with dementia, promoting independence.
  • Maintain knowledge of relevant legislation, including the Mental Health Act 1983, the Mental Capacity Act, NICE Dementia Guidelines and the National Dementia Strategy.
  • Review regularly with the clinical lead the overall nature of the workload, work methods and techniques, and evaluate clinical practice as a whole.
  • Undertake effective risk assessment and support other staff to recognise and de‑escalate difficult, challenging and aggressive behaviour, ensuring all patients on the caseload have up‑to‑date risk assessments and risk management plans.
  • Act at all times in accordance with the Nursing and Midwifery Council Code of Professional Practice.
  • Ensure an effective workload management system is in place and that there is an equitable distribution of workload in liaison with the Team Manager.
  • Participate in multi‑disciplinary and multi‑agency decision‑making processes through team meetings, team building and team maintenance.
Qualifications & Experience
  • Registered or newly qualified Band 5 Mental Health Nurse.
  • Experience working with dementia patients in a residential or community setting is desirable.
  • Strong knowledge of dementia care guidelines and legislation.
  • Excellent communication, organisational and multi‑agency working skills.
