Job summary

HCRG Care Group is seeking a Band 5 Community Staff Nurse to support Lancashire Health Visiting and School Nursing in the Skelmersdale area. You will work predominantly in families' homes, delivering the healthy child programme and collaborating with SCPHN and multi-disciplinary teams.

The role requires NMC registration, a driving licence, and a passion for improving child and family health within the local community.

Qualifications

• Be a registered nurse with NMC registration and a passion for improving the lives of children and families within the local community.
• Experience in health promotion/public health activities is desirable.
• Ability to act independently as well as part of a multi-disciplinary team.

Responsibilities

• Deliver the healthy child programme at four levels as delegated by the SCPHN.
• Support leadership and delivery of a quality function and supervise as per guidelines.
• Be trauma aware and understand the impact of Adverse Childhood Experiences on children and families.
• Identify situations where referral to SCPHN is necessary.
