We are seeking a motivated and compassionate Band 6 Community Care Coordinator to join our Community Mental Health Team in Bradford. This is an exciting opportunity to make a real difference in the lives of service users by delivering high‑quality, person‑centred mental health care.
As a Care Coordinator, you will manage a defined caseload, working autonomously and collaboratively within a multidisciplinary team. You will be responsible for assessment, planning, intervention, and evaluation of care, ensuring that service users receive safe, effective, and recovery‑focused support.
