A local authority in the UK is seeking a Community Assessor to work closely with residents and professionals, conducting person-centred assessments and developing effective care plans. Ideal candidates will have experience in health or social care settings, strong communication skills, and the ability to work independently. You’ll carry out person-centred assessments in people’s homes and in the community, working closely with social workers, occupational therapists, and health professionals to develop effective care and support plans. Your work will make a real difference in improving outcomes and promoting independence for adults and their carers.
You will be responsible for writing detailed care and support plans for individuals with eligible needs, ensuring your assessments are clearly evidenced. As part of this process, you will make appropriate referrals for financial assessments and complete comprehensive risk assessments to support safe and effective care planning.
This role follows a 7-day fortnight rota, meaning you’ll work:
You’ll have experience of working with vulnerable adults or within a health or social care setting, with a good understanding of community support networks and the principles of person-centred care. Strong communication skills, empathy and the ability to work independently are essential. You’ll also need to be confident using IT systems and have excellent organisational skills to manage your caseload effectively. You’ll be part of a team that relies on each individual to play their part to the best of their ability.
