This is a Relief, Sessional/Relief vacancy that will close in 25 days at 23:59 BST.

The role

We’re looking for someone to work on in our Hospital Discharge team. You’ll be working closely with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community.

Job Requirements
  • You’ll complete tasks to help customer get home safely from hospital such as doing property inspections, collecting belonging, dropping off food parcels, or arranging quotes for deep cleans so discharge can go ahead.
  • You’ll carryout a holistic assessment with customers and using an outcome star to complete an action plan to ensure you support the customer to help them stay happy, safe and well in their home.
  • You’ll do ad hoc or regular visits to people identified in the community as needing further support and complete tasks as requested. Examples of tasks might be checking homes are warm and safe, checking medication, food and other essentials are available.
  • You will establish links within the voluntary and community sector to support a holistic approach and tailored to the customer needs.

  • Ability to work under pressure and manage your daily tasks.

  • Hold a full UK driving license with use of your own vehicle including business insurance.
  • DBS required.
