• GPs hold patient lists, supporting continuity of care and long-term relationships with patients and families.
• Collaborate with Advanced Nurse Practitioners, Clinical Pharmacists, nurses, Care Co-ordinators and Social Prescribers to manage workload and ensure quality care.
• Participation in MDT meetings and practice discussions to develop services and improve patient access.
