We are seeking a dedicated Clinical Team Leader to join our Primary Health Care team across APMS and CHPL for the next 12 months.

This role is pivotal in ensuring the delivery of high-quality, responsive care to our diverse patient population.

Main duties of the job

Your key responsibilities will be to lead and manage a multidisciplinary primary care team with a commitment to high standards of professional conduct and patient care and to act as a clinical expert, providing support and guidance to colleagues and students. Qualifications
  • Demonstrable experience as a Registered Health Care Professional, RGN/RN/RMN, Diploma Nursing Studies
  • Evidence of ongoing continuing professional development/Portfolio
  • Mentorship qualification and/or evidence of supervision/teaching/team management
  • Professional Competencies in required clinical skills pertaining to Primary Care Nursing
Knowledge
  • Current Legislation and impact on community nursing services.
  • Evidence based practice
  • Knowledge of Primary Care Nursing and Inclusion Health, with an awareness of Trauma Informed Practice
  • To demonstrate an understanding of positive team working and integration
  • Understanding of audit processes, Key Performance Indicators and Targets and QOF.
  • Demonstrate an understanding of Integrated Team working and partnership working with other CHCP teams
  • Understanding of research methodology and audit
  • To have an understanding of the principles of restorative practice and coaching
  • Knowledge of local health and social care service provision
Experience
  • Demonstrable experience of working at a senior nurse level
  • Experience in planning and providing care to patients within sexual health services including specialist services
  • Significant experience working with a bio-psychosocial approach within a multidisciplinary team
  • Developing and leading practitioners to deliver client responsive, trauma informed services
  • Substantial experience of patient management within MDT working
  • Experience of collaborative working across organisation both statutory and voluntary services
  • Experience of initiating and implementing clinical or professional change and evaluation of practice
  • Experience in using IT in clinical practice
