Job summary

Qualifications

• Registered with the General Pharmaceutical Council - GPhC.
• Excellent communication and interpersonal skills.
• Experience working in primary care, PCNs, or GP surgeries.

Responsibilities

• Undertake structured medication reviews for patients with long-term conditions.
• Assist with repeat prescription authorisation and medication queries.
• Provide expert advice on clinical medicines management to colleagues and patients.

Skills

Excellent communication and interpersonal skills

Strong clinical knowledge and attention to detail

Ability to work both independently and as part of a multidisciplinary team

Experience in LTC such as Diabetes, Hypertension, Asthma

Education

Registered with the General Pharmaceutical Council - GPhC

Independent Prescriber qualification or willingness to work towards

Familiarity with System One

Experience working in primary care, PCNs, or GP surgeries
