Job summary

FedBucks is recruiting a Clinical Pharmacist to join our expanding in-house pharmacy service across Buckinghamshire. You will work with GPs, nurses and technicians to optimise medicines and deliver patient-centred care across multiple practices.

The role offers 37.5 hours per week (Mon-Fri), with part-time consideration, an NHS Pension, and pro rata annual leave. Based at Penn Surgery with travel to PCN sites, with a focus on high-quality prescribing and service development.

Qualifications

• Registered Pharmacist with GPhC.
• Independent Prescriber qualification or willingness to undertake training.
• Experience in primary care, community or hospital pharmacy.
• Excellent clinical knowledge and evidence-based prescribing.

Responsibilities

• Deliver structured medication reviews for patients with complex needs and polypharmacy.
• Manage long-term condition clinics (asthma, COPD, hypertension, etc.).
• Lead medicines optimisation reviews for care home residents and housebound patients.
• Support proactive care planning and multidisciplinary team meetings.
• Provide expert medicines advice to patients, carers and colleagues.
