Virtual Pharmacist Ltd is seeking experienced Clinical Pharmacists to join our national contractor network supporting GP practices and PCNs remotely. This is a flexible, self‑employed role ideal for pharmacists with proven GP practice experience, offering the opportunity to deliver high‑quality clinical services from home.

You will undertake structured medication reviews, acute and chronic disease support, discharge reviews and wider medicines optimisation tasks as part of established multidisciplinary teams. Main duties of the job

The contractor will provide a range of pharmacy‑led clinical services to GP practices, which may include: conducting Structured Medication Reviews, reviewing acute prescription requests, processing hospital discharge and outpatient letters, supporting long‑term condition management, triaging and managing patient queries, delivering medicines optimisation interventions, and contributing to QOF, IIF and local incentive targets. You will work remotely using NHS‑compliant systems provided by Virtual Pharmacist, collaborating with GPs, nurses and wider MDT members to improve patient outcomes, reduce clinical workload pressures and ensure safe, effective prescribing within primary care.

Job responsibilities
Clinical responsibilities
  • Conduct Structured Medication Reviews (SMRs) for patients with complex polypharmacy or long‑term conditions.
  • Review and process acute prescription requests, ensuring safe and evidence‑based prescribing.
  • Action and manage hospital discharge summaries, outpatient letters and other clinical correspondence.
  • Provide medicines optimisation support, including deprescribing, monitoring, and safety interventions.
  • Support chronic disease reviews as part of the multidisciplinary team (e.g., hypertension, asthma, diabetes, heart failure).
  • Manage patient queries relating to medicines, minor ailments and condition self‑management.
  • Provide prescribing advice and guidance to practice teams within your clinical competence.
  • Work towards QOF, IIF and local incentive framework targets as required by the practice.
  • Document all clinical work to a high standard within practice protocols and NHS information governance requirements.
Operational & professional responsibilities
  • Work autonomously and safely while adhering to practice policies, NICE guidance and local prescribing pathways.
  • Collaborate with GPs, nurses, practice managers and wider MDT members to support efficient clinical workflows.
  • Use EMIS Web or SystmOne to manage tasks, conduct reviews and document outcomes.
  • Escalate clinical concerns appropriately and seek senior clinical support when needed.
  • Maintain professional registration, CPD and clinical competence in line with GPhC requirements.
  • Follow all Virtual Pharmacist clinical governance and safeguarding policies.
  • Participate in peer support, professional development sessions and reflective practice when required.
Equipment & support
  • A secure NHS‑compliant clinical laptop, smartcard access support (where applicable) and all required software will be provided.
  • Contractors have access to Senior Clinical Pharmacist advice for clinical queries.
  • Virtual Pharmacist provides operational guidance, workflows, and access to clinical resources.
Contract & working pattern
  • Self‑employed contractor role.
  • Engagements typically last 12 months with the option to extend by mutual agreement.
  • Remote working from anywhere in the UK.
  • Flexible sessions available Monday to Friday.
  • Part‑time contractors must provide a minimum of 18 hours per week.
