Virtual Pharmacist Ltd is seeking experienced Clinical Pharmacists to join our national contractor network supporting GP practices and PCNs remotely. This is a flexible, self-employed role ideal for pharmacists with proven GP practice experience, offering the opportunity to deliver high-quality clinical services from home. You will undertake structured medication reviews, acute and chronic disease support, discharge reviews and wider medicines optimisation tasks as part of established multidisciplinary teams. The contractor will provide a range of pharmacy-led clinical services to GP practices, which may include: conducting Structured Medication Reviews, reviewing acute prescription requests, processing hospital discharge and outpatient letters, supporting long‑term condition management, triaging and managing patient queries, delivering medicines optimisation interventions, and contributing to QOF, IIF and local incentive targets. You will work remotely using NHS‑compliant systems provided by Virtual Pharmacist, collaborating with GPs, nurses and wider MDT members to improve patient outcomes, reduce clinical workload pressures and ensure safe, effective prescribing within primary care.

The Clinical Pharmacist will work with GP practices and Primary Care Networks to provide pharmacy‑led clinical services that support safe, effective and efficient patient care. This role is fully remote and requires strong experience within general practice. Sessions and working patterns will be arranged in advance based on practice needs and contractor availability.
