Job summary

A leading independent healthcare provider in Nottingham seeks a Clinical Pharmacist to conduct medication reviews and provide patient advice. Responsibilities include supporting safe prescribing practices and collaboration in the Pharmacy Department. Overview

We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department. This is a bank role working on an "as and when required" basis, you will be required to supplement staffing levels during busy periods and provide cover for existing staff in times of annual leave or absence.

Responsibilities
  • Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
  • Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
  • Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
  • Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
  • Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
Qualifications
  • A degree in Pharmacy MPharm, BSc or BPharm.
  • GPhC registration as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
  • Excellent communication and clinical decision-making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Confident using electronic prescribing and medication administration systems (e.g., EPMA).
