A leading healthcare organization in Weymouth is seeking a Clinical Pharmacist to enhance medication management within primary care. The ideal candidate will have a master's degree in Pharmacy, registration with the General Pharmaceutical Council, and experience in working with multidisciplinary teams. An exciting opportunity has arisen for a Clinical Pharmacist to be part of our growing team supporting the Weymouth & Portland Primary Care Network(PCN). This role is funded under the AdditionalRoles & Responsibilities Scheme. It is an interesting and varied role, working directly with other PCN pharmacists, pharmacy technicians and the GP leads to develop the structured medicationreview (SMR) and medicines optimisation services within and across the five GP Practices of our PCN.
With an established team in place, comprising experienced pharmacists and pharmacy technicians, this could be an excellent opportunity for a newly qualified pharmacist to bring their recent education and on-going enthusiasm to a primary care role.
Most work will be undertaken from a central location within Weymouth but travel between the network practices will also be necessary. The successful candidate will have a role in managing patient's long-term conditions as well as promoting self-care.
The candidate will have excellent written and verbal communication skills, experience of influencing others, good clinical knowledge and an understanding of prescribing data.
The successful applicant will work as part of a multidisciplinary team, working closely with all primary care colleagues with input and support from general practitioners, to deliver Structured Medication Reviews (SMRs), management of medicines on transfer of care and systems for safer prescribing and other key roles as outlined in the Network DES minimum role requirements.

The post holder will work within their clinicalcompetencies as part of a multi-disciplinary team to provide expertise inclinical medicines management, provide structured medicationreviews, manage long term conditions, management of medicines on transfer ofcare and systems for safer prescribing, manage repeat prescriptionauthorisations and reauthorisation, acute prescription request, whileaddressing both the public health and social care needs of patients in the GPpractice(s) that make up the PCN.
The post holder will contribute to qualityimprovement and clinical audit activities as well as supporting delivery of the Integrated Care System\'s Prescribing Quality Scheme.
This role is pivotal to improving the quality of care andoperational efficiencies so requires motivation and passion to deliver anexcellent service within general practice.
The post holder will be supported to develop their roleto become a non-medical independent prescriber, after completion of the PCPEP, if thatqualification is not already held.
