Job description

Stevenage Healthcare Ltd are supporting Stevenage South PCN to recruit an experienced clinical pharmacist to support the practices on a part-time basis. This position is offered on a part-time basis for one day a week - Friday 9am - 5pm. Working as an independent prescriber, the successful applicant willsupport the multi-disciplinary team, in line with the strategic objectives of the PCN.

Main duties of the job
  • Implementing an effective medicine management system within the practices, including cost-effective prescribing.
  • Providing prescribing advice and support for both patients and colleagues.
  • Undertaking clinical medication reviews with patients.
  • Managing caseload of patients with common/minor/self-limiting ailments.
  • Reconciling medicines following discharge from hospitals, intermediate care and into care homes.

Experience
  • Ability to plan, organise and co-ordinate work effectively under their own initiative
  • Ability to interpret, understand and analyse financial information
  • Ability to plan and manage multiple workload priorities and meet deadlines
  • Effective time management and skills in prioritisation
  • Relevant experience of general practice IT systems
  • Driving license and access to own vehicle
  • Previously worked within a similar position within the public sector desirable.
