A primary care facility in Essex is seeking a qualified and experienced clinical pharmacist for a full-time role. The successful candidate will work within a multidisciplinary team to enhance medication management and improve patient outcomes. Responsibilities include conducting medication reviews and supporting chronic disease management. Applicants should have a Master's degree in Pharmacy, GPhC registration, and strong communication skills. This role also requires proficiency in clinical systems like SystmOne or EMIS and a commitment to patient-centred care.
Hassengate Medical Centre is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on a full-time basis of 37.5 hours a week.
The clinical pharmacist will work as an integral part of the general practice team within the Primary Care Network to optimise the use of medicines, improve patient outcomes, reduce waste, and support the management of long-term conditions. They will provide clinical leadership on all aspects of medicines management within the practice and support safe, evidence-based, and cost-effective prescribing. The postholder will work closely with GPs, nurses, and other healthcare professionals to provide high-quality, person-centred care to patients, contributing to the overall efficiency and quality of clinical services.
Hassengate Medical Centre is situated in Stanford-le-Hope, Essex. We have 13, 250 registered patients and a CQC rating of good.
