Job Summary

You will be employed as a pharmacist acting within professional boundaries, working alongside a team of pharmacists in general practice and supported by a senior clinical pharmacist and GP who will mentor and support your development. You will take responsibility for areas of work identified by the PCN, provide primary support to general practice staff with prescription and medication queries, support the repeat prescription system, handle acute prescription requests, and conduct medicines reconciliation on transfer of care. You will also provide clinical leadership on medicines optimisation, manage aspects of the quality and outcomes framework, and participate in PCN quality improvement projects.

In this role you will ensure practice integration with community and hospital pharmacy to utilise skill mix, improve patient outcomes, increase access to healthcare, and manage workload. Your motivation and passion for delivering excellent service will be essential to improving quality of care and operational efficiency.

About Us

Conexus Healthcare is a community interest company and the federation for general practice in Wakefield. It is part of the Wakefield Health Alliance South Primary Care Network (WHA South), which comprises five GP practices located in the south of the district: White Rose Surgery, The Grange Medical Practice, Park Green Surgery, Langthwaite Surgery and College Lane Surgery. Collectively, we deliver health care services to approximately 65,000 registered patients.

WHA South is a forward‑thinking and supportive PCN that encourages personal development and growth. We are committed to improving patient health care with a particular focus on inequalities across our network footprint. The PCN team comprises GPs, nurses, pharmacists, technicians, care coordinators, physiotherapists, health and wellbeing coordinators, and mental health professionals, fostering an MDT approach to patient care and the vision of neighbourhood health. We already employ a successful pharmacy team and are looking to expand further with an additional pharmacist who will be supported in delivering excellent care and support for both our patients and staff within our busy surgeries.

The role holder will be required to work at any of the five member practices within the South Network.
